This policy explains what information this website collects and what we do with it. It covers this website only.
It is not our Notice of Privacy Practices. That is a separate document describing how we handle protected health information in the pharmacy, as required by HIPAA. Nothing on this website is a substitute for it.
This site does not accept prescription refills, transfers, or any patient health information. Our refill links take you to RxLocal (patient.rxlocal.com) and its mobile apps, which are operated by a third party under their own privacy policy. Anything you enter there is governed by that policy, not this one.
The only form on this site is the clinic request form, for organizations asking us to hold a vaccination clinic. If you submit it, we receive the organization or group name, your name, your email address and phone number, the type of clinic, an estimated number of people, a location, preferred timing, and any message you add.
We use that information solely to contact you and arrange the clinic. Please do not enter health information about yourself or anyone else in that form — it is not the right place for it, and we do not want it there.
We count page views so we know which pages are useful. When you load a page, the page path and a timestamp are sent to our own server, and our web server records the usual request information: IP address, browser type, and the page that referred you.
This information is kept on servers we control, is used only in aggregate to understand site traffic, and is not used to identify individual visitors.
This website is not directed at children and we do not knowingly collect information from them. If you believe a child has sent us information through this site, contact us and we will delete it.
You can browse this site without giving us any information. If you have submitted the clinic request form and want the information removed, contact us and we will delete it and confirm in writing.
If we change what this website collects or how we use it, we will update this page and revise the effective date above.
